Enrollment Verification Request

An enrollment verification form is used only by current students of Columbia College Hollywood to verify your existing college enrollment for purposes such as health insurance.  It is not to be used to order transcripts.  To request a transcript, please use our Transcript Request Form.

Enrollment Verification requests can be made either online, by fax, email or in person.  The fastest way to order an Enrollment Verification is by completing our Online Enrollment Verification Form.  If you prefer to order an Enrollment Verification using a Paper Enrollment Verification Form, you may print out a paper form and mail, fax or deliever it in person to the Office of the Registrar.  There is no fee for an enrollment verifcation.

To ensure that no person can obtain a copy of your Enrollment Verification without your permission, we require you to provide the last 4 digits of your Social Security number and either your written signature on our paper enrollment verification form or your electronic signature on our online enrollment verification form.
